『Screws in Orthodontics: My Essential Co-Worker』
（矯正治療におけるネジ：私の重要な協力者）

“Screws in Orthodontics: My Essential Co-Worker” will provide a comprehensive exploration of the pivotal role screws play in contemporary orthodontic treatment, particularly in the management of complex malocclusions. The lecture will focus on clinical approaches, demonstrating the practical utility of screws in various orthodontic scenarios, supported by carefully selected case studies.
The first segment will address the use of screws in the correction of deep bite, illustrating the treatment process, challenges, and the significant improvements achieved in vertical dimension and occlusal function. The subsequent discussion will examine open bite correction, with clinical cases highlighting the efficacy of screws in re-establishing proper occlusion and functional bite relationships.
The lecture will also explore the management of severe dental crowding, showcasing how screws facilitate space creation and alignment in cases where conventional approaches may be insufficient. Additionally, the application of screws in re-treatment cases will be presented, with an emphasis on their ability to address relapse or complications resulting from previous interventions, ultimately ensuring long-term treatment stability and optimal outcomes.
Through an integration of clinical experience and evidence-based practice, this lecture aims to underscore the indispensable role of screws in enhancing the precision and efficacy of orthodontic treatment, offering valuable insights for practitioners seeking to refine their clinical techniques and achieve superior results.
〈Career〉
- i graduated from dental school in Korea in 2008.
- from 2008 to 2011, I completed my orthodontic specialty training and obtained my certification as a dental orthodontic specialist, recognized by the Korean Association of Orthodontists (KAO) and certified by the Ministry of Health and Welfare of the Republic of Korea.
- after finishing my residency course and fellowship, I have been exclusively practicing orthodontics in a private clinic since 2012.
- additionally, I serve as an adjunct professor in the Department of Orthodontics at Ewha Womans University.
- i have been honored to receive the Outstanding Case Award from the Korean Association of Orthodontists (KAO) in both 2019 and 2022.
- i graduated with a master’s degree from Ewha Womans University in 2011. My thesis measured the airway in patients who underwent orthognathic surgery for Class III malocclusion using CBCT.
- i completed my Ph.D. at Yonsei University College of Dentistry, where I developed a 3D rendering program and authored a paper on 3D soft tissue measurement.
- based on my research and active involvement in the KAO, I received the “Awards for New Ph.D.” in 2023.
- i am currently serving as a Key Opinion Leader (KOL) for SureSmile Global/APAC under DentsplySirona.

補綴治療を含めた、包括的矯正治療の実践においては、従来の矯正学と補綴をベースにした咬合論の限界にしばしば遭遇する。
これは、咬合学がそれぞれ別の専門分野や対象患者が大きく異なったことに起因している様に感じる。
包括的矯正治療においては、これら現代までに確立されてきた咬合論を再考して、新たな学問として発展させることが確立させることが急務であると考えられる。
そこで、今回私が新たに提唱する生体最適化咬合論BOOCを取り入れたアップデートした新JIOS包括的矯正歯科難易度分類とそれらに基づいた包括的矯正治療戦略的ガイドラインに関して会員に向けて解説する。
A Novel Framework for Categorizing Orthodontic Treatment Complexity Involving the Biologically Optimized Occlusion Concept

In the practice of comprehensive orthodontic treatment, including prosthetic treatment, we often encounter the limitations of conventional orthodontics and occlusion theories based on prosthodontics. This seems to stem from the fact that occlusion studies have developed separately in different specialized fields and have been applied to distinct patient populations.
In comprehensive orthodontic treatment, it is imperative to reconsider these established occlusion theories and develop them into a new academic discipline. Therefore, in this presentation, I will introduce the updated New JIOS Interdisciplinary Orthodontic Difficulty Classification, incorporating the Biologically Optimized Occlusion Concept (BOOC) that I propose, along with a strategic guideline for comprehensive orthodontic treatment based on this classification and provide an explanation to the members.

「オルソインプラントセラピーの実践」

人生100年時代と言われる昨今、患者の生涯におけるQOLを向上させるには、患者の口腔内のQOLを向上させることが大切であることは言うまでもない。そのため、我々歯科医師は、患者の口腔内QOLを向上させるため、炎症のコントロール、力のコントロール、審美性の向上を可及的に低侵襲におこなわなければならない。そして、インプラント治療、矯正治療は、歯の切削をほぼ行わない低侵襲な治療法といえる。このインプラント治療と矯正治療を併用して行う治療法を、オルソインプラントセラピーと称し、筆者は日常臨床において実践している。
日本人の不正咬合の割合は多く、たとえ1本の前歯部欠損であっても、適正なスペースが保たれていることは少なく、それ故に、ほとんどの症例で、インプラント治療を行う前処置として矯正治療が必要となる。そして、臼歯部に欠損がある症例の場合、適切なバーティカルストップや矯正のアンカーを確立するために、矯正治療前にインプラントの埋入が必要となる場合もある。インプラントは一度埋入すると位置の変更が難しいため、インプラント治療を埋入するタイミング、ポジションは極めて難しいものとなる。
そこで今回は、日常臨床において矯正治療とインプラント治療を併用する上でのポイントを、筆者の経験を踏まえて解説していきたい。

Practice of Ortho-implant Therapy

In today’s era of 100-year lifespans, it goes without saying that it is important to improve the patient’s oral QOL in order to improve the patient’s lifetime. Therefore, in order to improve the patient’s oral QOL, we dentists must control inflammation, control force, and improve aesthetics as minimally invasively as possible. Implant treatment and orthodontic treatment can be said to be minimally invasive treatments that almost do not involve cutting teeth. This treatment method that combines implant treatment and orthodontic treatment is called orthoimplant therapy, and I practice it in my daily clinical practice.
The rate of malocclusion in Japan is high, and even if one anterior tooth is missing, it is rare that proper space is maintained, so in most cases, orthodontic treatment is required as a pretreatment for implant treatment. And in cases where there is a defect in the molar, implant placement may be required prior to orthodontic treatment in order to establish an appropriate vertical stop or orthodontic anchor. Since it is difficult to change the position of the implant once it is placed, the timing and position of implant treatment are extremely difficult.
Therefore, this time, I would like to explain the key points in using orthodontic treatment and implant treatment together in daily clinical practice, based on the author’s experience.

「包括的⻭科治療におけるTMJの重要性」

包括的歯科治療における矯正治療は、診査・診断からブラケットポジション・ワイヤーシークエンスなど、一般歯科医にとってはハードルの高い治療であった。しかし、アライナー型の矯正装置が著しく普及してきた現在、一般歯科医における矯正治療が身近な治療となりつつある。
AIが発達してきた今日、診査・診断は様々な分析ソフトを用いることにより歯科医師であれば、誰でも容易に行える時代となった. 骨格分析により、症例が抜歯なのか、非抜歯なのかが判断できる。ある程度の上顎前突症例では、アライナー型矯正装置の登場により、上顎の遠心移動も容易になり、症例によっては非抜歯症例へと難易度を下げることが可能となった。
しかし、矯正治療の分析において、下顎の位置異常があった場合には誤診へとつながる。
顎機能検査やMRIにて、顎運動や関節円板の確認を怠ると、下顎位が大きく移動し治療結果に大きな影響を与える可能性がある。過去に、レベリングを開始し抜歯しなければ良かった症例を経験している歯科医師も多いのでは無いだろうか？
そこで本講演では、長期間にわたり歯列を安定・維持させるために必要な知識として、TMJの基礎から顎関節症への対応、顎位が大きく変化し治療計画変更を行なった症例を提示し、解説します。

The Importance of Temporomandibular Joint in Comprehensive Dental Treatment

Orthodontic treatment in comprehensive dentistry has been a highest level for general dentists in terms of examination, diagnosis, bracket position, and wire sequencing. However, now that aligner orthodontic appliances have become remarkably popular, orthodontic treatment in general dentists is becoming more accessible.
Today, with the development of AI, diagnosis and examination can be easily performed by any dentist by using various analytical software. Skeletal analysis can determine whether a case is extraction or non-extraction. The advent of aligner-type orthodontic appliances has facilitated centrifugal movement of the maxilla in cases with a certain degree of maxillary protrusion, and in some cases has made it possible to reduce the difficulty level to non-extraction cases.
However, in the analysis of orthodontic treatment, abnormal mandibular position can lead to misdiagnosis. Failure to confirm the jaw motion and the articular disk by jaw function tests and MRI may result in a significant shift of the mandibular position, which may have a significant impact on the treatment outcome.
Lot of dentists may have experienced cases in the past in which leveling was initiated and the tooth should not have been extracted.
In this lecture, I will explain the basics of TMJ, how to deal with temporomandibular joint disorder, and cases in which treatment plans were changed due to significant changes in jaw position, as knowledge necessary for the long-term stability and maintenance of the dentition.
