患者の口腔内の健康を長期的に維持・増進するためには、顕在化している問題だけでなく潜在的な問題点も含めて精査し、包括的な治療計画を立案することが重要である。生理的な治療顎位を模索し、各歯の予知性を評価するとともに、歯周組織の炎症をコントロールしたうえで、安定した咬合を確立する必要がある。
症例によっては保存的・補綴的治療のみで対応可能な場合もあるが、矯正治療、さらには外科的矯正治療を含めた包括的介入が求められることもある。
近年、デジタル技術の進展により精密な治療が可能となっているが、その応用には従来のアナログ的コンセプトの十分な理解が不可欠である。具体的には、①フェイシャル・ドリブン、②生理的下顎位、③炎症のない歯周組織、④適正なアンテリア・ガイダンス、⑤安定した咬合を維持するための咬合面形態および咬合接触の確立、が重要な要素として挙げられる。
これらを達成するためには、単なるMI（Minimal Intervention）ではなく、個々の症例に応じた最適な介入であるOI（Optimal Intervention）の概念に基づき、治療オプションを適切に選択・組み合わせることが重要である。
本発表では、長期的に安定している症例を提示し、治療オプションの選択および介入の順序について検討するとともに、後戻り防止の観点から、アンテリア・ガイダンスおよび臼歯部咬合面の確立の重要性について考察する。

Treatment Strategies and the Importance of Occlusion in Comprehensive Oral Rehabilitation

To achieve long-term maintenance and enhancement of oral health, it is essential to conduct a thorough assessment not only of evident problems but also of underlying, non-apparent issues, and to formulate a comprehensive treatment plan. Establishing a physiological mandibular position, evaluating the prognosis of each tooth, and controlling periodontal inflammation are prerequisites for achieving a stable occlusion.
Depending on the case, some conditions can be managed with conservative and prosthetic treatments alone, whereas others may require orthodontic treatment or even orthognathic surgery as part of a comprehensive approach.
With the advancement of digital technologies, highly precise treatments have become feasible. However, their effective application requires a solid understanding of conventional analog-based concepts. These include: (1) a facially driven approach, (2) a physiological mandibular position, (3) inflammation-free periodontal tissues, (4) appropriate anterior guidance, and (5) proper occlusal morphology and contacts to maintain stable occlusion.
Achievement of these goals requires not only adherence to Minimal Intervention (MI) but also the application of Optimal Intervention (OI), in which treatment options are selected and combined according to the needs of each case.
Several cases with favorable long-term stability are presented. The selection and sequencing of treatment options are analyzed, and the importance of anterior guidance and posterior occlusal morphology in preventing relapse is discussed.

偉大なる先達から紡がれてきた近代歯科学は、デジタル・テクノロジーによって形を変えながら急成長している。
思い描いてきたデジタル・クローニングが現実のものとなり、近未来歯科はデジタル・デンティストリー無くしては語れないフェーズへと進んでいる。
事実、デジタル・テクノロジーを駆使した『矯正治療』や『補綴治療』においては、CBCTとIOSの併用による診断および臨床がメインストリームとなり、高い評価を得られるようになってきた。
一方で、デジタル・テクノロジーに対する「アレルギー」や「ストレス」から、一部では従前の状態に後退りする事象も散見され、アナログとデジタルの見えない溝をどう扱うべきなのかという難題も出てきた。
そこで今回は、咬合再構成の主軸となる『矯正』と『補綴』にテーマを絞り、『デジタルテクノロジーを駆使した近未来歯科のグランドビジョン（基本構想）』を見出すべく、壮大な冒険を繰り広げてみたいと考えている。

“ADVENTURES: The Integration of Digital Orthodontics and Digital Prosthodontics”

Modern dentistry, built upon the foundational achievements of our distinguished predecessors, is undergoing rapid transformation and expansion driven by digital technology. The long-envisioned concept of digital cloning has materialized into reality, advancing near-future dentistry into a phase where the paradigm of digital dentistry is an absolute prerequisite.

Presently, within the realms of digitally-driven orthodontic and prosthodontic treatments, diagnostic and clinical workflows utilizing the combined application of Cone Beam Computed Tomography (CBCT) and Intraoral Scanners (IOS) have established themselves as the mainstream, garnering significant clinical acclaim.

Conversely, a reversion to conventional methods is occasionally observed in certain sectors, stemming from practitioners’ resistance or “aversion” to adapting to new digital technologies. This phenomenon highlights a complex, emerging challenge: how to effectively bridge the unseen divide between analog and digital methodologies.

Therefore, this presentation will focus specifically on orthodontics and prosthodontics—the fundamental pillars of occlusal reconstruction. By navigating this specialized focus, we aim to embark on an expansive exploration to formulate a “Grand Vision for Near-Future Digital Dentistry.”

前歯部治療は審美的でかつ機能的である事が必要不可欠である。また、低侵襲である事が患者にとって望ましいと考えられる。保存不可能な歯において、最終補綴装置を考慮しないで抜歯してしまう事は、その後の治療に大きな影響を与えてしまう可能性がある。そのため、機能性、審美性を考慮し、低侵襲で治療する選択肢として抜歯即時インプラントは有効と考える。今回、上顎前歯部に対して抜歯即時インプラントにて修復治療を行なったので、若干の考察を含め報告する。
患者は55歳女性。転倒にて上顎前歯部の外傷による補綴装置の破折にて来院。下顎前歯部叢生のため、上顎前歯部へ早期接触を認めていた。上顎中切歯の歯冠歯頸部の水平破折の診断にて保存困難と判断し、抜歯即時インプラントで修復処置を行なった。下顎前歯部叢生に対しアライナー矯正を行い、上顎両側中切歯歯頸部歯肉ラインの非対称に対して、歯根破折歯のエクストルージョンを行った。その後、抜歯即時インプラントを埋入し、骨移植を行った。唇側歯肉退縮部はマイクロスコープ下で結合組織移植術を行った。歯肉安定後、プロビジョナルレストレーションにて歯頸部歯肉を修正し最終補綴装置を装着。
術後3年経過したが補綴装置にチッピングや破折はなく、X線写真上でも骨吸収像はなく経過良好である。

“A case of a single missing tooth in the maxillary central incisor using implant”

Anterior tooth treatment must be both aesthetic and functional. Furthermore, minimally invasive treatment is desirable for the patient. Extracting teeth that cannot be saved without considering the final prosthesis can significantly impact subsequent treatment. Therefore, considering functionality and aesthetics, immediate implant placement after extraction is considered an effective minimally invasive treatment option. This report describes a case of restorative treatment using immediate implant placement for the maxillary anterior teeth, including some discussion points.

The patient was a 55-year-old woman. She presented with fracture of her prosthetic device due to trauma to her maxillary anterior teeth following a fall. Due to crowding of the mandibular anterior teeth, premature contact with the maxillary anterior teeth was observed. A horizontal fracture of the crown and cervical portion of the maxillary central incisor was diagnosed, making preservation impossible, and immediate implant placement was performed. Aligner orthodontics was performed for the mandibular anterior crowding, and extrusion of the fractured tooth was performed to address the asymmetry of the gingival line at the cervical portions of both maxillary central incisors.
Subsequently, an immediate implant was placed after tooth extraction, and bone grafting was performed. Connective tissue grafting was performed under a microscope on the labial gingival recession. After gingival stabilization, the cervical gingiva was corrected with a provisional restoration, and the final prosthesis was fitted.

Three years have passed since the treatment, and there has been no chipping or fracture of the prosthesis, and no bone resorption is visible on X-rays; the progress is excellent.

歯科において色彩学はほとんどの歯科大学でカリキュラムには含まれていないと報告されており、歯科審美において重要な要素であるにもかかわらず、軽視されがちな項目であると認識している。
臨床上、特に包括的矯正歯科治療においては審美的に非常に繊細な患者が多く、その要望もそれぞれであり、口腔内の限られたスペースでの製作を余儀なくされる中、歯牙形態の付与のみではその要望に答えることが困難なことが多く、色彩を使用した視覚効果を利用するなどして補綴臨床をこなしている。
そこで今回は色彩科学から人が色を認識する眼のメカニズム、色光と色材の根本的な違いと特徴、色の伝達に必要な測色の分類と種類、補綴歯科で利用できる色の視覚効果とその表現方法について実際の症例を参考にお伝えする。

“Color in dentistry”

It has been reported that color science is not included in the curriculum of most dental universities, and despite being an important element in dental aesthetics, it is often overlooked.

Clinically, especially in comprehensive orthodontic treatment, there are many patients with very sensitive aesthetic needs, and their requests vary. Given the limited space in the oral cavity, it is often difficult to meet these requests by simply creating tooth shapes, and prosthetic practice is carried out by utilizing the visual effects of color.

Therefore, this paper will discuss the mechanism of how the human eye perceives color, the fundamental differences and characteristics of colored light and colorants, the classification and types of colorimetric measurements necessary for color transmission, and the visual effects of color that can be used in prosthetic dentistry and how to express them, using actual case studies.
